EndoFest® Registration Information
EndoFest registration includes access to all general sessions an d didactic breakout sessions, as well as the exhibits, learning center, two receptions and lunch on Saturday. All participants must be registrants of EndoFest (overall conference fee) to register for hands-on sessions or add-on courses.
Registrations received on or before Thursday, August 20 will receive the early bird discount.
Registration online: ASGE strongly recommends that you register online. If you register online, your spot in each session will be automatically reserved. Register online at www.asge.org/endofest.

Please note that space is limited in hands-on sessions and your registration in each session can only be confirmed upon receipt of your registration form and payment. Cancellation and Change Policy
ASGE plans its hands-on sessions and related supplies for EndoFest based on registration. For that reason, requests for refunds of any kind must be received in writing no later than Friday, August 28, 2015 to email@example.com. Refunds requested before this date will be subject to an administrative fee of $100. Refunds will not be issued after September 4, 2015. Attendee substitutions are permitted with ASGE authorization. ASGE has the right to cancel a program at any time at its discretion.
Registration for EndoFest sessions is limited. Please do not reserve your overnight accommodations or make airline reservations until you have received a formal confirmation of registration from the ASGE office.

ASGE has arranged a room block for this course at $259 per night plus applicable taxes and fees. This discount is only valid if the reservation is made before August 24, 2015. Continuing Medical Education
The American Society for Gastrointestinal Endoscopy (ASGE) is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians. ASGE designates this live activity for a maximum of 23 AMA PRA Category 1 Credits™ (including the MOC and Bariatrics course). Physicians should claim only the credit commensurate with the extent of their participation in the activity. The MOC course will offer 3.75 CME credits, the Bariatrics course will offer 4.5 CME credits and EndoFest will offer up to 14.75 CME credits including hands-on sessions. Additional credit may be claimed separately for viewing DVDs through the ASGE Learning Center on-site.
General Information and Continuing Education for Nurses
All nurse participants will receive a certificate of attendance specifying hours of education accredited for AMA PRA Category 1 Credit™. This certificate can be used toward nursing requirements.
ASGE Conflict of Interest Policy
ASGE requires all individuals who are in a position to control the content of an educational activity to disclose all relevant financial relationships with any commercial interest, which is any entity producing, marketing, re-selling, or distributing health care goods or services consumed by, or used on, patients. Disclosures are made in written form prior to the start of the educational activity and any potential conflicts of interest that exist are resolved prior to the start of the activity through the ASGE Conflict of Interest Resolution Policy Process. All disclosures are made available and communicated to the learner prior to the activity starting.
