The ASGE (American Society for Gastrointestinal Endoscopy) Endoscopic Training Grant Award 2023 presented a unique and prestigious opportunity, affording me the privilege of engaging in a comprehensive training program. The focal point of this initiative was a five-week immersive experience in Japan, where I received specialized training in endoscopic submucosal dissection (ESD) under the expert guidance of Dr. Yutaka Saito, a distinguished authority in the field.

ESD stands as an advanced endoscopic methodology employed for the resection of early cancers and submucosal lesions within the gastrointestinal (GI) tract, encompassing the esophagus, stomach and colon. My training took place at the esteemed National Cancer Centre (NCC) in Tokyo, where Dr. Saito serves as the director of the endoscopy division at the Tsukiji campus.

The tenure at NCC was marked by a diverse array of experiences contributing to the enhancement of my proficiency and comprehension of ESD. This encompassed firsthand observation of numerous ESD cases and hands-on practice utilizing animal models. Each day commenced with sessions dedicated to endoscopic and preoperative conferences, focusing on the macroscopic appearance of lesions and their corresponding pathological characteristics across the esophagus, stomach and colon.

The NCC facilities provided dedicated spaces for both endoscopic surveillance and ESD procedures. Witnessing three to five ESD cases daily exposed me to the intricacies of these procedures. Moreover, under the mentorship of Dr. Saito and Dr. Takamaru Sensei, I commenced the development of a case report, an original article and a review article.

My professional interests lie comprehensively in the domain of endoscopic submucosal dissection. My intention is to seamlessly integrate the acquired techniques under Dr. Saito's tutelage into my future clinical practice. This endeavor holds substantial promise, particularly considering the current absence of ESD practitioners in my home country, Bangladesh. Upon my return, I aspire to pioneer ESD initiatives and establish programs to disseminate knowledge among gastroenterologists.

Acknowledging the invaluable support from ASGE, Dr. Saito, Dr. Takamaru and the NCC staff, I express gratitude for this exceptional opportunity. The exposure gained and the knowledge acquired during this program have been instrumental in expanding my clinical expertise. Presently, I am involved in ESD procedures in my home country, albeit on a limited scale due to resource constraints. Nevertheless, I am poised to leverage the skills and insights obtained at NCC to enhance the quality of ESD in my country, thereby addressing the unmet medical needs of those requiring such treatment.

In summation, the ASGE Endoscopic Training Grant 2023 has been transformative for me both professionally and personally. It has equipped me with invaluable skills and knowledge in ESD, empowering me to contribute meaningfully to the advancement of health care practices in my home country. I am profoundly appreciative of this opportunity and remain committed to the continuous improvement of ESD treatment for the benefit of those in need.
