CY2026 Tri-Society Coding Update

ASGE, ACG, and AGA released their jointly developed 2026 CPT Coding Updates for Gastroenterology. This tri‑society resource provides gastroenterologists, endoscopy units, and billing professionals with consolidated guidance on new, revised, and deleted codes affecting GI and endoscopic services in the 2026 calendar year. Key highlights include:

  • New Category I CPT codes like 43889 (Endoscopic Sleeve Gastroplasty, ESG) now signal broad clinical adoption, supporting payer advocacy for coverage as a standard bariatric/endobariatric benefit.
  • G2211 add-on for complex/prolonged E/M in outpatient settings—payable when visits exceed 20 minutes or involve comorbidities, counseling, and care coordination for chronic GI conditions (IBD, liver disease, obesity).
  • E/M Refresher: Select outpatient E/M level (99202–99215) by MDM or total time; document explicitly for chronic GI cases like IBD/obesity.
